Jockey Club Affordable Outpatient Services Subsidy Scheme (All quota be used)

Introduction:
With many General Out-patient Clinics of the Hospital Authority being converted to designated COVID-19 clinics or suspending their services, the Jockey Club Affordable Outpatient Services Subsidy Scheme aims to provide affordable outpatient services to eligible non-COVID-19 patients in non-profit-making and private clinics. This will enable those who cannot afford private outpatient services to continue getting access to diagnosis and treatment. The scheme will last for six months starting from 24 March 2022, with approximately 33,000 eligible patients benefiting. Lok Sin Tong will be the coordinator for service provision for Yau Tsim Mong District.

Service Scope: 
The Scheme primarily serves patients with episodic medical needs. Eligible persons can receive medical consultation with basic medication for three days at a fee of $50. The Hong Kong Jockey Club Charities Trust will provide a subsidy of $250 and the rest of the cost to be covered by the clinics. If it is medically necessary to prescribe medication outside the subsidy scope (e.g. medication for more than three days or special drugs), the doctor will discuss with the patient and seek his prior agreement.

Service Target:
Non-COVID-19 patients living in Yau Tsim Mong District, who fulfill any one of the following criteria will be eligible for participating in the Scheme:
(1) Elderly aged 65 or above; 
(2) Children aged 12 or below;
(3) Persons aged 13 to 64, with proof of appointments at General Out-patient Clinics of the Hospital Authority in the past 12 months; 
(4) With financial difficulties as referred by Lok Sin Tong:
     i. Low-income families who receive School Textbook Assistance (Full grant or Half grant);
     ii. Recipients of Comprehensive Social Security Assistance scheme (CSSA);
     iii. Recipients of Old Age Living Allowance (OALA)
     iv. Recipients of Disability Allowance ( High and Normal are also included) ;
     v. Low-income families referred by networked non-profit making social service units,
        referral criteria include individuals living in crowded spaces (such as subdivided room),
        single parent family, EM family and people with mental illness.

Remarks: Persons in need may check with Lok Sin Tong on the referral eligibility. To benefit more patients in need, patients who have joined Hospital Authority General Out-patient Clinic's Public-Private Partnership Scheme are not eligible for participating in this Scheme.

Service Registration:
Eligible persons should call the clinic to make an appointment, and indicate they are eligible for the Scheme. There is a daily service quota for each clinic, which will be allocated on a first-come, first-served basis. On the day of the consultation, please bring the following documents according to the eligibility categories:
