Project Abstract:
Funded by the Hong Kong Jockey Club, The Lok Sin Tong Benevolent Society, Kowloon collaborates with five organizations: Jockey Club CADENZA Hub, The Tung Wah Group of Hospitals, St. James’ Settlement, Hong Kong Society for Rehabilitation and the Family Planning Association of Hong Kong, joining hands in running a pilot program since July 2022 which target at female aged 50 to 64, namely Jockey Club “Shining Journey 50+” Women Wellness Programme.
The programme take reference from the Integrated care for older people (ICOPE) of the World Health Organisation, a guidance for person-centred assessment and pathways in primary care. The care plan include multiple interventions by a range of professionals to manage declines in intrinsic capacity, provide social care and support, support self-management and support caregivers.
Program Objectives:
To optimise females’ intrinsic capacity and to establish a healthy lifestyle through a range of Educational Courses, Intervention Programme and Experiential Workshops in accordance with the result of Health Screening Questionnaire.

Women who are from 50 to 64 years old, living in Kowloon City, Yau-Tsim-Mong & Tsuen Wan districts are all welcomed.
