Statement of Purposes of Personal Data Collection

1. Purpose of Collection

The personal data provided by service users with whom this medical unit interacts in the delivery of services and other related activities will be used by the Medical Service Department of The Lok Sin Tong Benevolent Society, Kowloon (hereinafter known as The Department) for the delivery of services, statistical and research purposes, and other related matters. The provision of personal data is voluntary. If the service users do not provide sufficient information, this medical unit and The Department may not be able to provide certain or any services.

   2. Classes of Transferees

The personal data provided are mainly for internal use within The Department (including this clinic) for the purposes mentioned in paragraph 1 above; however the personal data may also be disclosed to other relevant parties, if required, where the service users’ consent has been given to such disclosure, or where such disclosure is allowed under the Personal Data (Privacy) Ordinance.

   3. Access of Personal Data

Service users have a right to request access to and to request the correction of their own personal data under sections 18 and 22 and Data Protection Principle 6, Schedule 1 of the Personal Data (Privacy) Ordinance. A fee may be imposed for complying with a data access request.
