Yau Tsim Mong District Health Centre (Operating Organization: Lok Sin Tong Benevolent Society, Kowloon)
Since its establishment four years ago, the Yau Tsim Mong District Health Centre Express (operated by The Lok Sin Tong Benevolent Society, Kowloon) has served as a hub for district health services and resources. In alignment with government policy directions and the development of the Primary Healthcare Blueprint, the Centre has been dedicated to promoting primary healthcare services within the district, ensuring that citizens with different health needs receive appropriate and diversified support, while gradually fostering a prevention-oriented health culture.
To meet community needs and further enhance service quality, the Yau Tsim Mong District Health Centre Express was officially upgraded to the Yau Tsim Mong District Health Centre on 27 October 2025. The core centre, located at 1047 Canton Road, Mong Kok, occupies approximately 1,000 square metres, tripling the size of the original Express. The upgraded centre features new rehabilitation facilities and an audiometry and vision assessment room, as well as improved amenities for health education activities. In addition, three satellite centres have been set up in Prince Edward, Yau Ma Tei, and Jordan (the latter to commence operation in January 2026) to provide residents with convenient access to services in their neighbourhoods.

Service Scope
The Yau Tsim Mong District Health Centre provides citizens with key primary healthcare services, including health promotion and education, health risk assessments, and chronic disease management, with the aim of establishing a district-based primary healthcare model. A professional healthcare team provides members with free health risk assessments, case management, and nursing care coordination, as well as assistance in matching them with suitable family doctors to manage chronic diseases.
Adhering to the principles of being district-based, public-private collaboration, and medical-social cooperation, the centre connects healthcare providers in Yau Tsim Mong and nearby districts (including Family Doctors, Nurses, Chinese Medicine Practitioners, Physiotherapists, Occupational Therapists, Optometrists, Dietitians, Speech Therapists, Podiatrists and Laboratory) to deliver comprehensive primary healthcare services through an interprofessional approach.
Meanwhile, the YTMDHC collaborates with community partners to coordinate social services and health-related services within the district, thereby integrating comprehensive community medical and social resources for citizens.
Service Highlights
Chronic Disease Co-Care Pilot Scheme
The Chronic Disease Co-Care Pilot Scheme (CDCC Pilot Scheme) has been launched since 13 November 2023. Under the scheme, eligible persons will be subsidised by the Government to undergo screening by a family doctor of their own choice. Those who are diagnosed with prediabetes, diabetes mellitus (DM) or hypertension (HT) will be taken care of by their family doctor with long-term management including medical consultations and drug prescription as clinically indicated. Starting from March 28, the scope of screening under the CDCC Pilot Scheme will be expanded to include blood lipid testing in addition to DM and HT screening to achieve full coverage of the "three highs".
Life Course Preventive Care
Life Course Preventive Care is an evidence-based, comprehensive health strategy that emphasises on disease prevention and personalised needs. It provides guidance on the health needs of citizens across different life stages. Health management requires active participation from citizens, as well as support from Family Doctors and community healthcare professionals in disease prevention to enhance citizens' self-management ability.
Family Doctor
The family doctor is the major primary care service provider who provides comprehensive, family-centric, continuing, preventive, and coordinated care to the citizen and their family members. Apart from treating acute and chronic diseases, a family doctor who have a good understanding of the citizen’s health conditions and needs, also plays a crucial role in continuously supporting the citizen in the prevention and self-management of diseases, providing suitable care and professional advice to promote the overall well-being.
